Efficacy of laparoscopic hernia repair combined with Nissen fundus folding in the treatment of patients with gastroesophageal reflux complicated with hiatal hernia

Abstract:

Objective

To analyze the clinical efficacy of laparoscopic hernia repair combined with Nissen fundus folding in the treatment of patients with gastroesophageal reflux complicated with hiatal hernia.

Methods

Eighty patients with gastroesophageal reflux complicated with hiatal hernia in the Department of Gastroenterology of Baogang Hospital of Inner Mongolia from July 2019 to May 2020 were selected as the research subjects. They were divided into group A (40 cases) and B (40 cases). Group A received laparoscopic herniorrhaphy with Nissen fundus folding, and group B received laparoscopic herniorrhaphy with Dor fundus folding. The operative conditions and esophageal reflux were compared between the two groups , esophageal pressure, digestive disease quality of life index (GLQI) grade, gastroesophageal reflux disease curative effect (GERD-Q), recurrence rate, and adverse reactions occurred.

Results

There was no statistical significance in the operation situation between the two groups (P>0.05). Three months after operation, the percentage of reflux time, times, long reflux times and acid reflux time were significantly improved, and the improvement was more obvious in group A (P<0.05). The mean value of esophageal residual pressure, lower sphincter pressure, relaxation rate and invalid swallowing rate were significantly improved, and the improvement was better in group A than in group B (P<0.05). GLQI and GERD-Q scores were improved in both groups, and the GLQI score in group A was higher than that in group B, while the GERD-Q score was lower than that in group B (P<0.05). There was no statistical significance in adverse reactions and recurrence rates between the two groups (P>0.05).

Conclusion

Laparoscopic hernia repair combined with Nissen fundus foldus has a significant effect on the inhibition of reflux and postoperative dysphagia, which not only improves the pressure of the lower esophageal sphincter, but also improves the lives of patients, which is safe and effective.
