Application of deperitonealization of the round ligament in female inguinal hernia repair

Abstract:

Objective

To investigate the safety and efficacy of deperitonealization to preserve the round ligament of uterus in laparoscopic transabdominal preperitoneal repair (TAPP) for inguinal hernia in female patients.

Methods

From January 2013 to July 2019, a total of 75 female patients underwent TAPP in Shaanxi Provincial People's Hospital were retrospectively analyzed. All patients were treated by achieving deperitonealization to preserve the round ligament of uterus. The baseline data, intraoperative and postoperative conditions were observed and analyzed.

Results

All patients underwent surgery successfully. The operation time of unilateral hernia was (80.0±19.3) min, the postoperative hospital stay was (2.3±0.9) days, and the VAS score on the first day after surgery was (2.1±0.8). There were no labia majora edema and numbness or hematoma formation. There were 6 cases of postoperative seroma, all recovered without any treatment after 2 months follow-up. The follow-up time was 3~84 months, and the median follow-up time was 37 months. There was no hernia recurrence or mesh infection in all patients.

Conclusion

In TAPP surgery for female inguinal hernia patients, when the round ligament of uterus is loosely connected to the hernia sac and peritoneal space, deperitonealization of the round ligament of uterus could be achieved. It is a simple operation with less complications, and an ideal treatment to preserve the round ligament of uterus in TAPP for inguinal hernia in female patients.
