目的

对比观察使用3D腹腔镜联合自固定补片在成人腹股沟疝腹腔镜腹膜前修补中的效果。

方法

回顾性分析合肥市第二人民医院2019年9月至2022年1月共80例腹股沟疝患者资料,依使用腹腔镜不同分为3D腹腔镜组和2D腹腔镜组,观察两组患者行腹腔镜腹膜前疝修补术的住院时间、手术时间、术后疼痛等情况。

结果

3D腹腔镜组与2D腹腔镜组的手术时间分别为(68.35±11.48)min和(74.47±12.56)min,采用3D腹腔镜联合自固定补片较普通腹腔镜行TAPP手术时间更短,两组间差异有统计学意义(P<0.05)。术后住院时间、自主活动时间、肠鸣音恢复时间3D腹腔镜组分别为(5.12±1.38)d、(24.56±3.68)h、(7.48±2.18)h,2D腹腔镜组为(5.04±1.44)d、(23.89±3.54)h、(7.58±2.13)h,两组比较差异无统计学意义(P>0.05)。两组的疼痛视觉模拟评分在术前,术后1 d、1周、1个月、3个月比较,差异均无统计学意义(P>0.05)。

结论

成人腹股沟疝采用3D腹腔镜联合自固定补片腹膜前修补可以缩短手术时间。

Objective

To compare and observe the effect of 3D laparoscopy combined with self-fixation mesh in adult laparoscopic transabdominal preperitoneal inguinal hernia repair (TAPP).

Methods

The data of 80 adult patients with inguinal hernia in the Second People's Hospital of Hefei from September 2019 to January 2022 were analyzed retrospectively. They were divided into 3D laparoscopy group and 2D laparoscopy group according to different laparoscopy. The hospitalization time, operation time, postoperative pain and other conditions of the patients undergoing TAPP in the two groups were observed.

Results

The operation time of 3D laparoscopy group and 2D laparoscopy group was (68.35±11.48) min and (74.47±12.56) min, respectively. The operation time of TAPP with 3D laparoscopy combined with self-fixation mesh was shorter than that with conventional laparoscopy, and there was a statistically significant difference between the two groups (P<0.05). Postoperative hospitalization time, autonomous activity time and bowel sounds recovery time were (5.12±1.38) d, (24.56±3.68) h, (7.48±2.18) h in the 3D group and (5.04±1.44) d, (23.89±3.54) h, (7.58±2.13) h in the 2D group, respectively. There was no significant difference between the two groups (P>0.05). There was no significant difference in pain visual analogue scale scores between the two groups before operation, 1 day, 1 week, 1 month, and 3 months after operation (P>0.05).

Conclusion

3D laparoscopy combined with self-fixation mesh for TAPP in adults can shorten the operation time.
