探讨硬膜外麻醉下短暂腹腔镜探查判断腹股沟嵌顿疝内容物存活情况的临床价值。

回顾性分析2017年1月至2018年12月苏北人民医院疝外科35例腹股沟嵌顿疝开放手术患者术中，因疝内容物存活情况可疑，而行硬膜外麻醉下短暂腹腔镜探查的临床资料，经腹腔镜探查后，根据探查结果行相应的手术。

35例腹股沟嵌顿疝患者在硬膜外麻醉下术中短暂行腹腔镜探查，疝内容物为肠管，均顺利完成手术，术中1例探查后发现肠坏死，中转全身麻醉开腹行坏死肠段切除术，然后简单行内环修补。其余34例探查发现嵌顿肠管颜色均恢复红润，且蠕动可，术中均行腹股沟疝开放无张力修补术。随访1年均无复发。

对于腹股沟嵌顿疝疝内容物存活情况可疑患者，在硬膜外麻醉下短暂行腹腔镜探查，可以安全有效地探查腹腔，观察疝内容物情况，为嵌顿疝临床治疗提供了思路。

To explore the clinical value of short-term laparoscopic exploration under epidural anesthesia in judging the survival of the contents of incarcerated inguinal hernia.

A retrospective study was conducted in 35 patients with incarcerated inguinal hernia who underwent open surgery from January 2017 to December 2018 in Department of Hernia Surgery, Northern Jiangsu People's Hospital. They underwent short-term laparoscopic exploration under epidural anaesthesia due to suspicious survival of hernia contents in open surgery. Surgical operations were carried out according to the results of the laparoscopic exploration.

Short-term laparoscopic exploration under epidural anaesthesia was performed for 35 incarcerated inguinal hernia patients. The hernia contents were bowel. The operations on the 35 patients with inguinal incarcerated hernia were all successful. There was 1 case with intestinal necrosis after laparoscopic exploration. General anesthesia was used for laparotomy of necrotic intestinal resection, and simple internal ring repair was performed. The remaining 34 cases with intestinal function recovery underwent open tension-free hernia repair after laparoscopic exploration. All patients were followed up for 1 year, no recurrence was found.

In incarcerated inguinal hernia patients with suspicious survival of hernia contents, short-term laparoscopic exploration under epidural anaesthesia can be used to explore abdominal cavity safely and effectively, and to observe the situation of the hernia contents, providing ideas for clinical treatment of incarcerated hernia.
