目的

对腹腔镜下应用生物补片修补直肠术后盆底缺损的安全性和有效性进行总结和分析,为盆底缺损的治疗提供相对合理的方案。

方法

选取2017年1月至2021年12月,首都医科大学附属北京中医医院61例直肠术后盆底缺损采用腹腔镜下腹腔内生物补片植入修补法治疗的患者,记录患者手术前后的临床参数,观察并分析其术后并发症及复发情况。

结果

61例患者均经腹腔镜顺利完成手术,手术时间40~100 min,平均76 min;术中出血量10~70 ml,平均40 ml;术后住院时间3~11 d,平均6 d。术后出现盆腔积液1例,经穿刺抽液后治愈;腹胀3例,均于5 d内自行缓解;肠梗阻1例,于术后第10 d治愈;肺部感染2例,经抗感染治疗后治愈;戳孔感染1例,给予积极换药处理后治愈;患者术后均未出现肠瘘、腹腔感染、戳孔血肿、戳孔疝、慢性疼痛等严重并发症。随访时间6~54个月,平均16个月,均未出现会阴疝、肠梗阻。

结论

直肠术后盆底缺损采用腹腔镜下生物补片植入修补法进行治疗,是一种相对有效且操作便捷、安全性高的治疗方法,可有效降低盆底缺损相关并发症的发生及疝的出现。

Objective

To analyze the safety and efficacy of biological mesh for pelvic floor re-construction after rectal surgery and to provide a relatively reasonable scheme for the treatment of pelvic floor defects.

Methods

From January 2017 to December 2021, 61 patients with pelvic floor defects after rectal surgery were treated by laparoscopic biological mesh in Beijing Hospital of Traditional Chinese Medicine, Affiliated with Capital Medical University. The clinical parameters before and after the operation were recorded, and the complications and recurrence were observed and analyzed.

Results

All 61 patients who underwent laparoscopic surgery were successful. The operation time ranged from 40 to 100 minutes, with an average of 76 minutes. Intraoperative bleeding volume ranged from 10 to 70 ml, with an average of 40 ml. The postoperative hospital stay ranged from 3 to 11 days, with an average of 6 days. There were 1 case of pelvic effusion after operation, which was cured by percutaneous aspiration; 3 cases of abdominal distension were alleviated within 5 days; 1 case of intestinal obstruction was cured within 10 days after operation; 2 cases of puncture infection were cured after positive dressing change; and no intestinal fistula, abdominal infection, puncture hematoma, or puncture hernia occurred after operation. Severe complications such as chronic pain. The follow-up period ranged from 6 to 54 months, with an average of 16 months. There was no recurrence of perineal hernia or intestinal obstruction.

Conclusion

Pelvic floor defects after rectal surgery are repaired by laparoscopic biological mesh, which is an effective, safe and feasible treatment method that can promote the rapid recovery of patients and reduce the incidence of complications and hernia recurrence.
